AGES & STAGES QUESTIONNAIRE

The ASQ is a developmental screening tool. Screening involves a quick assessment of your child’s current skills and risks for delays. We use two assessments: The ASQ-3 and the ASQ:SE-2.

ASQ-3 Assesses:

  1. COMMUNICATION SKILLS:

    How a child understands interactions and how they express themselves.

  2. FINE MOTOR SKILLS:

    Hand and finger movement and coordination.

  3. GROSS MOTOR SKILLS:

    Arm, leg, and other large muscle movement.

  4. PERSONAL/SOCIAL SKILLS:

    Self-help skills (like feeding and dressing) and interactions with others.

  5. PROBLEM SOLVING SKILLS:

    Thinking skills; how children play with toys and solve problems.

ASQ:SE-2 Assesses:

  1. AUTONOMY:

    Moving toward independence.

  2. COMPLIANCE:

    Following rules and directions.

  3. ADAPTIVE FUNCTIONING:

    Ability to cope with bodily needs (eating, toileting).

  4. SELF-REGULATION:

    Ability to calm, settle down or adjust to stimulation or conditions around them.

  5. AFFECT:

    Demonstrating feelings and empathy for others.

  6. SOCIAL-COMMUNICATION:

    Interacting with others by responding to or initiating communication to indicate interest, needs, or feelings.

FREQUENTLY ASKED QUESTIONS

  • WHO FILLS OUT THE ASQ?

    ASQ’s are designed for parents and caregivers to fill out. You know your child best!

  • IS IT DIFFICULT TO USE?

    No! An ASQ can be completed at home, over the phone, in person or even in a waiting room. The survey is made up of questions and fun activities that can be done with a child. It typically only takes 10-15 minutes to fill out.

  • HOW FREQUENTLY SHOULD MY CHILD BE SCREENED?

    California recommends screenings at 9 months, 18 months and 24-30 months (think 3 by age 3!) and regularly screening until age 5. The ASQ can screen children as young as 1 month and up to 66 months (5 1/2 years).

  • WHAT HAPPENS WHEN I RECEIVE MY CHILD'S SCORE?

    If there are no concerns about your child’s development, you may not receive recommendations for follow-up. If your child is struggling with some skills, we will help you navigate next steps, including a possible referral for further assessment.

  • SHOULD I CONTINUE SCREENING IF NO CONCERNS ARISE?

    Yes! It is still recommended that your child get screened at the next regular interval.

  • WILL THIS GIVE MY CHILD A LABEL?

    No. The ASQ is a developmental screening tool that helps you understand where your child is in their growth and development. It highlights your child's strengths and can also identify areas where additional support or further assessment may be helpful."
